While the general law does not allow Euthanisa like in the Netherlands, we are given the choice to choose to end our life in the face of a terminal illness diagnosis.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">AMD is a legal document to inform the doctor treating that you do not want to prolong your life with any extraordinary life-sustaining treatment in the event of the following condition:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Terminally ill&nbsp;<\/li>\n\n\n\n<li>Unconscious&nbsp;<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>How to make an AMD<\/strong>&nbsp;<\/h4>\n\n\n\n<ol class=\"wp-block-list\" start=\"1\">\n<li>Obtain an <a href=\"https:\/\/www.moh.gov.sg\/docs\/librariesprovider5\/forms\/form1amd(270905).pdf?sfvrsn=fc5eb19b_0\" target=\"_blank\" rel=\"noreferrer noopener\">AMD form<\/a>&nbsp;\n<ul class=\"wp-block-list\">\n<li>Physical form also available in Medical Clinics, Polyclinics and Hospitals&nbsp;<br><\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Consult a Doctor together with a Witness&nbsp;\n<ul class=\"wp-block-list\">\n<li>Doctor&#8217;s responsibility to determine that: \n<ul class=\"wp-block-list\">\n<li>You are not being forced into making the AMD.<\/li>\n\n\n\n<li>You are not mentally disordered.<\/li>\n\n\n\n<li>You understand the nature and implications of making an AMD.&nbsp;<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Witnesses (2 required)\n<ul class=\"wp-block-list\">\n<li>One will be the Doctor&nbsp;<\/li>\n\n\n\n<li>The other can be the Doctor\u2019s Nurse or any adult, age of 21 years old or above.&nbsp;<\/li>\n\n\n\n<li>AMD will be void if the witness is a family member or has demised&nbsp;<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Return the Completed Form to The Registrar of AMD&nbsp;\n<ul class=\"wp-block-list\">\n<li>Either by mail or by hand&nbsp;<\/li>\n\n\n\n<li>Must be returned in a Sealed Envelope&nbsp;&nbsp;<\/li>\n\n\n\n<li>Registrar of Advance Medical Directives at Ministry of Health, Singapore, College of Medicine Building, 16 College Road, Singapore 169854&nbsp;<\/li>\n\n\n\n<li>You will receive an Acknowledgement when the AMD have be registered with The Registrar of AMD&nbsp;<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n\n\n\n<p class=\"wp-block-paragraph\">AMD is a free legal document. You may check out on other forms and guidelines on AMD <a href=\"https:\/\/www.moh.gov.sg\/hpp\/all-healthcare-professionals\/guidelines\/GuidelineDetails\/advance-medical-directive\" target=\"_blank\" rel=\"noreferrer noopener\">here<\/a>.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>The Lasting Power of Attorney (LPA)<\/strong>&nbsp;<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">LPA will come in place when we lose our mental capacity to make decisions. The power will be granted to our donee to make decisions on our behalf.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Our Donee can be entrusted to make <strong>decisions<\/strong> in these 2 broad areas&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Personal Welfare: our daily activities and place of living etc..&nbsp;<em>and\/or&nbsp;<\/em><\/li>\n\n\n\n<li>Property &amp; Affairs: our financial matters e.g funds in bank accounts and property&nbsp;<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Singaporeans enjoy <strong>fee waiver<\/strong> until 31 March 2023 for <a href=\"https:\/\/www.msf.gov.sg\/opg\/AnalyticsReports\/LPA_Form_1_2020.pdf\" target=\"_blank\" rel=\"noopener\" title=\"LPA Form 1 Submission\">LPA Form 1 submission<\/a>.&nbsp;<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">What is the difference between an LPA Form 1 and LPA Form 2?&nbsp;<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>LPA Form 1 \u2013 Standard version to grant donees <strong>general powers<\/strong> with basic restrictions.&nbsp;<\/li>\n\n\n\n<li>LPA Form 2 \u2013 Use to grant donees with <strong>customised powers<\/strong>.
